Proposal Documents
- Indiana Commission for Higher Education (ICHE) Degree Program Proposal Form and Checklist.
- Use this document for all new degree and certificate programs.
(For certificates, only sections 1 though 2.c.i, plus section 4, are necessary) - Please ensure that the current form is being used, and it is subject to ICHE updating.

- Board of Trustees (BOT) Cover Sheet
- All new degree programs must be approved aat the Board of Trustees before advancing to ICHE. The BOT Cover Sheet is the sole document presented. Therefore, the proposing campus should prepare this document and include it with the program proposal at the time of submission within APPEAR.
- Course Articulation Planning for New Baccalaureate Degrees (Appendix 9)
- For all new baccalaureate programs, an articulation plan for students from Indiana's 2-year institutions (Ivy Tech Community College and Vincennes University) is required. Please complete the Appendix 9 document to be included with the proposal.

- Intent to Offer Financial Aid Eligible Certificate Program
- For new certificates to be offered with financial aid eligibility, this for is necessary for working with the campus Office of Student Financial Aid to add the certificate to the Program Participation Agreement with the Federal Student Aid division of the US Department of Education.

Documents for Online and Collaborative Proposals
- Office of Online Education Online Academic Program Initial Request Form (Qualitrics)
- Required for all online programming prior to proposal development.
- Includes all online degrees and certificates, collaborative online programs, hybrid programming of 50% or more online delivery, and all majors (including tracks, concentrations, and specializations) being offered within online degrees.
- New Degree Cover Sheet
- May be used as brief summary information in a proposal. The content of this document comprises the data entry in the APPEAR submission
